Job description

We are a specialist Physical Health Team that has recently expanded within Adult Community Mental Health Services in Black Country Healthcare NHS Foundation Trust. The team’s focus is to improve physical health outcomes for our patients, following standards issued by the Care Quality Commission, NHS England and NICE guidelines. This role involves cardio‑metabolic monitoring in outpatient clinics and occasional home visits to undertake annual cardio‑metabolic physical health checks, monitoring physical observations (BP, weight/height/BMI), completing ECGs and taking bloods (all training provided), and supporting senior clinicians within the one‑stop Clozapine Clinic.

Responsibilities
  • Provide support and time to an allocated group of service users to promote their recovery and physical health monitoring, maintaining them in their community environment and helping them build their own support network.
  • Support the individual service user to identify their needs and keep these at the fore at all times, working to the Recovery Model.
  • Support the individual service user with their physical health needs.
  • Support the ongoing development of the physical health clinics within community mental health services.
  • Support the clinical team in delivering high quality health care to the patients.
  • Support the running of Clozapine Clinics.
  • Undertake cardio‑metabolic monitoring in outpatient clinics and home visits.
  • Support senior clinicians within the one‑stop Clozapine Clinic.
Qualifications
  • Experience working with adults who have severe and enduring mental health difficulties.
  • Excellent communication skills and good IT skills.
  • Genuine interest in improving physical health for patients.
  • Ability to build good working relationships with the multidisciplinary team.
  • Capacity to help service users work towards goals within a recovery framework.
  • Must hold the national Care Certificate for clinical posts up to and including Band 4, or be supported to attain it once in post.
  • Willingness to undergo DBS checks if required.
Salary & Benefits

The salary is set in accordance with NHS Agenda for Change pay scales, based on experience working directly within the NHS. Experience gained with other NHS Trusts will be considered; candidates without prior NHS experience may be appointed at the lowest point unless they can demonstrate equivalent experience.
