Job description

Thestop smoking practitioner is responsible for supporting and facilitating thedelivery of smoking cessation programmes within the Place Based Local StopSmoking Service in Kent.

We haveopportunities for a new practitioner covering areas in Kent including West Kent(Tunbridge Wells, Tonbridge and Sevenoaks)

Face toface delivery in community locations.

Youwill receive your own cohort of patients and Face to Face clinics to supportand maintain and will help to improve the accessibility of Kents Place BasedStop Smoking Service through the pro-active promotion of the smoking cessationprogramme and referral pathways key health care providers, alliedprofessionals, community events, local agencies, and the general public.

Youwill be part of both the smoking cessation team. You will also receiveadditional in-house training including data security, first aid andsafeguarding.

Main duties of the job

The Ideal Candidate:would preferably have completed and be registered with the NCSCT as a Stop Smoking Practitioner already and have experience of working in this role.

Have strong interpersonal and communication skills.

Possess a minimum of 1 year experience working in a community-based smoking cessation service.

Are experienced in using behaviour change models within a work context.

Can demonstrate a clear understanding of the range and appropriate use of different smoking cessation interventions.

Display strong knowledge of the harmful effects of smoking, smoking as an addiction behaviour, withdrawal, and relapse.

Are flexible with good time management and planning skills.

Are interested in motivating and supporting both individuals in virtual and community-based settings.

  • Sound knowledge of delivering smoking cessation interventions and advising on smoking cessation to special populations e.g. people experiencing homelessness and Traveller communities.

Can raise public awareness and provision of consistent healthy lifestyle messaging, on issues relevant to the health of the community and in a multi-cultural setting.

Have experience in building strong links and relationships with a diverse range of groups, Organisations, and agencies.

Stop Smoking practitioner

MAIN PURPOSE

Our Stop Smoking Practitioners deliverone to one and group stop smoking interventions within a wide range ofsettings. Across Kent our service will includework within one to one and group settings and also involve evening/weekendwork.

The post holder will be required towork flexibly across the County as required ensuring their skills, expertiseand knowledge are fully utilised.

Thispost will:

Provide information to smokers to enable themto make an informed choice to make an attempt to stop smoking and then tosupport the smoker with a formal evidence based quit programme.

Develop and improve accessibility of the Kent PlaceBased Stop Smoking Service through the pro-active promotion of the smokingcessation programmes and referral pathways amongst primary and secondary careservices, other key health care providers, allied professionals, communitydevelopment initiatives, local agencies and the general public.

Provideongoing support to a caseload of referrals, supporting each quit attempt inline with national and company standards.

Contribute to the planning, delivery anddevelopment of the stop smoking service including specific project work.

This post will also:

In conjunction with the Delivery team leadplan and deliver an appropriate and accessible cessation service within thedelivery team area.

Provide cessation support including one to oneand group support, assessing clients suitability for Stop Smoking treatments.

Adhere to service protocols and procedures inrelation to the provision of supportive pharmacotherapy medication.

Collate accurate clinical records in line withnational OHID/NCSCT requirements and inputting data into the service web-baseddatabase in a timely and professional manner.

To support the service in delivering keyperformance indicators and activity as set out within the servicespecification.

Support the delivery of promotion activityacross Kent in line with the service marketing strategy.

Work flexibly to provide a coordinated serviceacross the localities to meet the needs of the client group. This will includework in the evening and some work on weekends where required.

Maintain up to date clinical knowledge,undertake evidence-based practice and promote the use of new and innovativepractice.

In conjunction with delivery team lead,conduct analysis and evaluation of designated programmes in order to improvethe quality of service.

Be responsible for identifying issues orconcerns to line manager, so that appropriate action can be taken

Contribute to ongoing needs assessment, auditand other governance programmes in relation to activities as part of the healthimprovement agenda.

To takeresponsibility for own professional development in accordance with the outcomesfrom the annual development review, in order to meet the requirements of theKnowledge and Skills Framework and appraisal system

Responsiblefor accurate uploading of own client data onto database whilst ensuring clientconfidentiality.

Ensurefollow up of clients is done in line with service protocol

Keepaccurate records in accordance with trust policy and data protection

Tomaintain an up to date electronic diary that is available to colleagues withinthe Stop Smoking team

To assistin producing reports and analysis of data that contribute towards improvingservice delivery and better client care

To utilisea range of complex motivational, influencing and negotiating skills to increaseclients motivation and adherence to stop smoking.

Responsiblefor personally identifiable client data as part of ongoing programme attendanceetc., working within the data protection policies.

Requiredto contribute to training, seminars and presentations across a range ofagencies and organisations.

  • Role incorporates evening / weekend working, with variable levels of travel across Staffordshire, working in a variety of settings including home visits, the community and community centres.
  • Full UK Driving licence and access to a car for work- with business purposes insurance.
  • Ability to travel across Staffordshire to areas where public transport may not be available.
  • Evidence of a personal commitment to continuing professional development and to maintaining an up-to-date professional knowledge supporting the development of business within the company.
  • This post involves working with vulnerable adults and/or young people as such the post holder will be required to apply for a disclosure of criminal records at an enhanced level (DBS).

Qualifications
  • Required:
  • Education to GCSE Standard at grades A-C for a minimum of 5 GCSEs (or equivalent) including Mathematics and English language or literature.
  • NCSCT Smoking Cessation Certified.
  • Training will be provided however health degree (or equivalent) is advantageous
Experience
  • Minimum of 1 year experience working in smoking cessation programmes across a range of service settings, including community, primary care and in patient settings.
  • NCSCT Accredited/Registered Stop Smoking Practitioner
  • Experience of working with adults, young people and children in a 1-2-1 and group environment, to support positive lifestyle change.
  • Experience of using behaviour change models within a work context
  • Experience of working in a multi-cultural setting with patients of different cultures, genders and ages.
  • Experience of working to time schedules.
  • Evidence of experience in embracing and implementing change to provide high quality health outcomes.
  • Experience of working to challenging performance targets and achieving excellent outcomes.
  • Sound knowledge of delivering smoking cessation interventions and advising on smoking cessation to special populations.
  • Knowledge of NICE guidance, NCSCT and PHE standards relating to the treatment of smoking cessation
