Job summary
Hollister Limited is seeking a Stoma Care Specialist or a Nurse with a passion for specialist community care to support a stoma care pathway from hospital to home. The role covers Wakefield and surrounding areas, offering a permanent contract with a 37.5 hour week and a Band 7 equivalent.

Qualifications
- Qualified Nurse RGN with current registration.
- Experience in community/district nursing is desirable.
- Recognised stoma care qualification is desirable.
Responsibilities
- Support patients discharged with a stoma in community settings.
- Provide safe, specialist nursing care and patient education post-surgery.
- Teach patients to manage their stoma independently.
- Maintain communication with acute teams, GPs, and community staff.
- Ensure accurate patient records are kept.
- Support introduction of new products and review with patients.
- Work with Nurse Manager to set annual objectives.
