A prominent healthcare provider in Waterlooville is seeking a dedicated Staff Nurse to join the Living Well Services team. The role involves supporting patients with life-limiting illnesses and their families, ensuring high-quality care within various community hubs. The Staff Nurse and Senior Staff Nurse Living Well Services (bank) role(s) will work within the Rowans Living Well Services, which are led by the Living Well Services and Community Engagement Manager who is responsible to Clinical Director. The role is based at Rowans Living Well Centre, Purbrook.
Rowans Hospice is a charity that provides free care and support to adults and their families living in the Portsmouth and South-East Hampshire region who have a life-limiting illness. Established in 1994, our income is generated by ongoing support of the local community, donations, legacies, fundraising and through our retail shops.
JobPurpose
The post holder is responsible forcontributing to the assessment and the implementation of programmes of care forusers of the Living Well Services and community hubs.
The post holder is also expected to carryout all relevant forms of care without direct supervision and to supervise,where necessary, other qualified, unqualified and volunteer staff within theLiving Well Services

Role Specifics
Inconjunction with the LWS Manager: provide a Monday to Friday 9-4 pm (Wednesdaytill 6pm) service including Community Satellite hubs
Tosupport the growing need for those who may look to The Rowans Hospice for helpand support before they reach the final stages of their life
Tosupport a service where there are opportunities to inform, support, care andre-able people, their families and friends, who are affected by the diagnosisof a life shortening or incurable illness or indeed to those who areapproaching the end of their natural life span.
Tosupport an environment that provides companionship for both the patient andcarer.
Tomaintain the health and well-being of the local community and as far as thismay be possible, support people in the process of Future planning and decisionmaking; helping them to remain in control of their lives for as long as thismay be possible
Ensureeach patient/carer is assessed /triaged and a programme of care and therapiesarranged to support their individual needs.
Toassess the needs of patients and carers using tools IPOS and CSNAT requiringintervention from other professionals/therapies within and outside the hospiceand refer appropriately.
Toliaise with members of the clinical team, Hospice Companions Co-Ordinator,Volunteers Service Manager, and relevant Primary Care and Social Careprofessionals to facilitate the continuity of patient care within thecommunity.
Toliaise with other in-patient/out-patient departments of hospitals asappropriate.
Inconjunction with The LWS Manager support the expansion and extension of currentservices, providing more therapy sessions for a greater number of people;offered flexibly throughout the week.
Topromote a therapeutic environment utilising the skills of therapist/artistsworking within the setting and taking an active part in special projects i.e.,Work/Mindfulness
Toprovide advice on symptom control/care management Living Well Services patientsto other health care professionals.
Toreport to the LWS Manager / Deputy LWS Manager /Hospice Doctor/Palliative CareHub any relevant information on the condition of specific patients, their careor treatment.
Toreport to the LWS Manager / Deputy LWS Manager / Director of Clinical Servicesany matter affecting the smooth running of the centre and community hubs
Toreport any complaints/safeguarding concerns from patients, their relatives, orothers acting on their behalf to the LWS Manager Deputy LWS Manager / Directorof Clinical Services and assist with investigation of complaints as necessaryknowing your boundaries and expectations within the process.
Toensure knowledge about patients/carers is maintained in the strictest confidence,and is not divulged, except to professionals where it would materially assistwith the programme of care or protection of the individual
Todemonstrate a consistent level of courtesy and consideration to patients, theirrelatives and visitors.
Topromote a high quality of care by adhering to set standards.
