Overview
An exciting opportunity for a Band 5 nurse to join a dynamic, motivated team of renal nurses who deliver a high standard of specialist outpatient care. You will be working in a friendly and supportive environment providing comprehensive care and support to dialysis‑dependent patients on our Dialysis outreach team.
Shift Details
Outreach team runs 7:00 – 19:30 Monday to Saturday, plus 12:00 – 18:00 on Sundays. The team is based on 7C/HDU and 7B.
Responsibilities
- Assess patients before dialysis.
- Calculate and set the dialysis prescription on the dialysis machine.
- Use ANTT technique as per Trust protocol.
- Access vascular access and commence dialysis treatment.
- Deliver prescribed IV medication via dialysis machine.
- Monitor patients during treatment.
- Troubleshoot alarms during treatment.
- Discontinue and disconnect the patient at the end of dialysis.
- Document treatment on CyberREN and other Trust systems.
- Clean the dialysis area as per Trust protocol.
- Prioritise patients based on clinical urgency.
- Support early discharge by ensuring outpatient dialysis is appropriately arranged.
- Contribute to reducing inpatient stay through proactive renal management.
- Maintain clear communication between the renal unit, ward staff, bed managers and the nephrology team.
- Liaise with the multidisciplinary team (nephrologist, pharmacists, dietitians, vascular access specialist nurses).
- Take responsibility and accountability for the assessment, planning, delivery and evaluation of nursing care.
- Undertake delegated tasks from the ward sister/charge nurse.
- After successful completion of the Foundation gateway, take charge of a ward/department for a span of duty.

Essential Criteria
- Recent clinical nursing experience as a student or a registered nurse.
- Current registration with the Nursing and Midwifery Council (NMC).
- BSc in Nursing (or equivalent Diploma in Nursing).
- Post‑registration qualification in a relevant specialty.
- Appropriate UK professional registration.
Desirable Criteria
- Basic IT skills, including e‑mail and word processing.
- Sound clinical nursing knowledge and a good understanding of nursing issues and clinical governance.
- Experience within a relevant clinical setting.
- Evidence of maintaining a clinical learning environment.
- Evidence of working with learners (pre‑registration or student nurses).
- Practice Assessor / Practice Supervisor training.
