Somerset NHS Foundation Trust is seeking a Registered Nurse to join the Discharge Lounge team. The role focuses on timely, safe patient flow and high‑quality care for adults within the hospital setting.

You will work 37.5 hours weekly, with shifts across 07:30–20:00 and 09:00–17:00 on weekends, supporting discharge processes, collaborating with multidisciplinary staff, and contributing to continuous service improvement.

Job summary

An exciting opportunity has arisen to join the highly motivated discharge lounge team in a Registered Nurse position.

The Discharge Lounge supports all areas within the Trust with an emphasis on maintaining timely and safe patient flow out of the Hospital.

The successful candidate will have experience of hospital care and a knowledge of patient discharge processes.

The role requires the individual to have enthusiasm and a passion to drive the service forward and meet service demands. They should have effective communication skills and an ability to support all the staff within the team to deliver high quality safe patient care.

Working Pattern: 37.5 hours per week during the hours of 07.30 - 20.00 Monday to Friday and 09.00 - 17.00 rotational weekends fairly shared between the team.

Main duties of the job

To provide and maintain a high standard of evidence based, patient centred care. To demonstrate good interpersonal skills and to develop practical skills required within the speciality, whist reflecting the philosophy of care.

To deliver patient care on a daily basis, using the systems in place in the department for a multi-disciplinary approach to patient-centred care delivery, which builds on the philosophy of greater involvement of patients and their relatives/carers by using the Trusts patient and public involvement strategy.

To provide effective and clinical support during patient's time in the Discharge Lounge. To provide clear instruction and guidance for those going home on to other healthcare settings

Support the systems for a multi-disciplinary approach to patient-centred care delivery, which builds on the philosophy of greater involvement of patients and their relatives/carers by using the Trusts patient and public involvement strategy

Delivers patient care including health promotion daily to patients who present with undifferentiated conditions including life-threatening emergencies to adults
