Job summary

Black Country Healthcare NHS Foundation Trust is advertising a specialist Speech and Language Therapist within the Assessment and Treatment to Independence in the Community Pathway. You will assess and support acute and/or complex communication needs and work closely with locality teams to enable smooth transitions and reduce inpatient stays.

Applicants should be HCPC registered, hold a degree in SLT, and have completed RCSLT competencies with band 5 experience.

Qualifications

• Degree in Speech and Language Therapy (SLT).
• HCPC registration is required.
• Completion of RCSLT competencies with band 5 experience.

Responsibilities

• Conduct formal and informal assessments using standardised tools.
• Provide direct and group therapy to individuals with complex communication needs.
• Document case notes and share findings with the MDT.
• Develop multidisciplinary care plans and coordinate with locality teams.
• Refer to other services as appropriate.
