Southend-On-Sea's Children, Young People and Families Public Health Service (CYP&FPH) are looking to recruit an enthusiastic and motivated practitioner with initiative and drive. Our service delivers high quality public health nursing, that promotes and improves health inequalities to children, young people and their families in Southend-On-Sea. You will need to have excellent communication and organisational skills, be willing to facilitate and support change in service delivery, have the ability to work autonomously and as part of an integrated skill mix team.
If this sounds like you, we have an exciting opportunity to work as a specialist school nurse for inclusion within Southend's CYP&FPH service.
You will work within a skill mix School Nursing team, working closely with partners, such as our Health Visitors, School Nurses, Schools, GPs and Social Care, providing seamless care to the children, young people and families of Southend. You'll also have an important role in recognising and protecting vulnerable children as well as using innovative ways to communicate with the Southend population such as ChatHealth and the use of social media.
The successful candidate will work within the School Nursing service ensuring the delivery of safe, effective and high-quality services to a caseload of children, young people and their families or carers, using research-based evidence to inform practice with agreed professional standards. The post will require the nurse to travel between homes, schools, and community hubs within the Southend area.

As a School Nurse working for Southend City Council you will have access to SAPHNA corporate membership, and you will be reimbursed for your NMC professional fees.

To work as an autonomous Specialist School Nurse to deliverthe Healthy Child Programme and tosupport the health and emotional well-being needs of Children and Young People(CYP) aged 5-19 who are Educated Other Than at School, including CYP who areattending pupil referral units, electively home educated (EHE), missing fromeducation and who are open to the local authority attendance team and assessedas having a identified health need requiring school nursing intervention.
To be the allocated named School Nurse for three AlternativeProvisions (AP) within Southend-On-Sea.
To work in partnership with awide range of professionals including but not limited to, GPs, Health Visiting,School Nursing, Community dental, Community paediatricians, Mental healthservices, Open Road, Supporting Families, Social Care, and the Youth offendingteam to ensure that children and young peoples health needs are supported, andsafeguarding is maintained.
To engage with safeguardingprocesses to ensure concerns are escalated through appropriate channels and asuitable plan of support can be developed to meet the need of the Child/Youngperson. This includes adhering to thelocal SET and national safeguarding policies and procedures, accessing mandatorysafeguarding supervision, liaising with the locality safeguarding team andsocial care, writing safeguarding reports and attending Child Protectionconference, core groups and Child in Need meetings.
Completion of health needs assessments to support in theidentification of health needs and inform a personalised plan ofcare/interventions in collaboration with the child, young person, and theirparent/carer.
To establish and maintain relationships with voluntaryagencies to support the understanding of health needs, increase accessibilityto services and address inequalities.
To support effective service responses and pathways wherethere are multi-agency service interfaces required.
To maintain an excellent standard of records for theservice.
It is recognised that children and young people within this client groupoften present with complex and/or enduring needs which require amultidisciplinary and flexible approach in the aim of recognising andaddressing barriers to services and reducing health inequalities. Due to thesefactors, children and young people in this client group may be at increasedvulnerability.
To promote to andupskill the Southend Children, Young People & Family Public Health service (0-19team) and other agencies regarding the health needs for this client groupincluding health inequalities and interventions.
