A healthcare service provider in Cannock is looking for a specialist to coordinate care for individuals facing housing issues related to substance use. The ideal candidate will provide harm reduction services, maintain a caseload, and collaborate closely with housing teams. Candidates should have a Foundation Degree or equivalent experience in related fields. This role offers a salary between £31,049 to £37,796 annually.

JOB PURPOSE
To provide a dedicated, comprehensive service to people experiencing homelessness, being at risk of homelessness and having substance related problems, or being at risk of relapse back into substance use and have a history of, or are at risk of disengaging from services. To also work closely with and provide advice and guidance to colleagues within the housing options team to integrate substance use intervention into care plans, increasing engagement and ongoing support, where appropriate.
