Job description

The post holder will deliver seamless care across organisational boundaries reviewing patients in a community setting in order to improve the ability for patients to self‑care; to enable patients to recover at home following illness/ preventing the need for patients to be admitted to hospital.

We are looking for an experienced physiotherapist who is highly motivated and a self‑starter with the ability to work as part of a multidisciplinary team and provide skills, knowledge and expertise in the care and management of frail older patients in a community setting. We provide a comprehensive assessment and rehabilitation programs for patients in their own homes and respond to the needs of the frail elderly population with a wide range of conditions including complex co‑morbidities, frailty, respiratory, musculoskeletal, and orthopaedic.

It is essential that you have a full UK driving licence and have a car available for work.
