By bringing together our expertise, we can improve patient care and enable local people to access the right support, care and treatment more easily than ever before.

This role provides rehabilitation support within the Community Neuro Rehabilitation Service, delivering neuro-rehabilitation interventions in outpatient, home, and community settings under the guidance of qualified therapists. The rehab assistant supports treatment across Physiotherapy, Occupational Therapy, and Speech and Language Therapy, managing a routine caseload and assessing and treating patients within agreed guidelines. They monitor patient progress, making minor adjustments to treatment programmes as appropriate, and work independently on a day-to-day basis while reporting progress and any concerns to the treating therapist. Regular guidance is sought from therapists to ensure safe and effective practice.

  • Record information accurately and timely.
  • To organise administrative tasks efficiently to support smooth and effective service delivery.
  • To engage in Continuing Professional Development (CPD) activities to maintain and enhance personal and professional competence.
  • Attend all relevant in-service training sessions to support ongoing skills development.
  • Participate fully in the SDHC appraisal process and developing a personal development plan aligned with the Knowledge and Skills Framework (KSF).
  • To ensure confidentiality at all times, only releasing confidential information obtained during the course of employment to those acting in an official capacity.
  • To comply with the Trust’s policies on diversity, the consumption of alcohol and non-smoking.
  • To report all clinical and non-clinical accidents or near misses promptly and, when required, to co-operate with any investigation undertaken.
