Jobbeschreibung

We understand that through purposeful and meaningful activity we can support people to rehabilitate, recover and self-care. Although your base will be on the ward, we understand that working beyond boundaries is the only way to holistically support people toward their goals to allow individuals safely home. We strongly believe that our therapy teams within our community hospitals play a vital role not just in the health and well-being of our patients, but for our wider communities as a whole. As your caseload will be varied, you will have the opportunity to work alongside individuals experiencing a range of long-term health conditions. A key aspect of this will involve assessment of frailty and providing meaningful support for people with palliative health conditions. This role is to be based in Johnson Community Hospital, Spalding Road, Pinchbeck, PE11 3DT.

  • To assess and develop personalised care plans for individuals admitted on to the ward
  • To (where appropriate) complete/delegate visits to individuals homes
  • To provide clinical leadership and supervision to members of the therapy team
  • To work with partner organisations in order to promote a 'discharge to assess' model of care
  • To work alongside the therapy leads in order to innovate and develop the therapy service
  • To demonstrate a commitment to personal development

Lincolnshire Community Health Services NHS Trust is in the top 25 per cent of all community trusts for overall staff engagementin the 2021 NHS National Staff Survey. At LCHS, we know diversity fosters creativity and innovation. We are committed to equality of opportunity, to being fair and inclusive, and to being a place where all belong. We therefore particularly encourage applications from candidates who are likely to be underrepresented in LCHS' workforce. LCHS recognises the increasing demand for more flexibility in working practices in order to accommodate the personal goals and commitments that employees experience at different stages during their working lives. Flexible working opportunities such as part-time working, compressed hours, job shares, and hybrid working models can help staff to balance their personal and work commitments. Arrangements can be put in place if it can be shown that the needs of the individual can be balanced with those of the service and the impact on other team members, enabling the Trust to recruit and retain skilled staff, raise staff morale, reduce absenteeism and respond to changing service needs more effectively.
