The Sutton Palliative Care Coordination Hub (PCCH) is recognised as a system-wide resource supporting transformational change across Sutton’s health and social care system.
The Hub serves as a single point of access for people aged 18 and over who are likely to die within 12 months. It provides a single referral point for care, coordination and delivery for professionals, individuals, families and carers, supported by nurse specialists who are experts in palliative and end of life care.
The team supports early identification and advance care planning and facilitates timely discharge from hospital through coordination with commissioned services and community partners.
The Hub is developing towards a stronger coordination, advisory and enabling model. Its purpose is to strengthen community capability and shared clinical ownership, rather than functioning primarily as a caseload-holding service. This post is central to that development and to embedding specialist palliative care within neighbourhood and community structures.
The Specialist Nurse for Palliative Care is a Band 8A clinical and operational leadership role within the Palliative Care Coordination Hub. The post holder will work across the two agreed PCN footprint sites (four neighbourhoods, two in each PCN footprint), ensuring that specialist palliative care expertise is consistently accessible within local multidisciplinary and community structures.
The role will complement the existing Band 8A Specialist Nurse for Palliative Care and Non-Medical Prescribing already in post. The two Band 8A roles will work as a senior clinical leadership pair,
providing mutual cross-cover during periods of absence and ensuring continuity of senior palliative care leadership, operational oversight and strategic development.
The post holder will retain a strong specialist palliative care focus while also influencing and developing the strategic direction and neighbourhood vision for the Hub. They will work with neighbourhood, PCN, Community Nursing, primary care, acute, hospice, social care and voluntary sector partners to embed the principles of the Hub into the four neighbourhoods and establish palliative and end of life care as core community business.
The post holder will be visible, accessible and solution focused, supporting teams to retain appropriate clinical ownership while providing specialist advice, coordination, education and leadership for people with palliative and end of life care needs.
