Job description

We are looking for a specialist nurse to join our care homes team. We provide support to 36 care homes across Rotherham borough. The specialist nurse will play a key role in supporting the delivery of high-quality person‑centred care across both nursing and residential settings. With a strong interest in driving continuous improvement, the successful candidate will work closely with care home staff to promote best practice, enhance clinical standards and contribute to positive resident outcomes. The role includes completing holistic and comprehensive clinical assessments, identifying care needs and providing expert guidance to ensure safe effective care is achieved. The specialist nurse will also act as a resource for education and support within the care home community.

Main duties of the job

The successful candidate will provide specialist nursing support to the care homes team, carrying out assessments and ensuring effective treatment plans for residents. They will assist in maintaining and developing quality assurance systems to monitor clinical standards across Rotherham care homes. The role also involves working with the local authorities contracting and quality team to promote learning and support quality improvement initiatives within nursing and residential care homes. The responsibilities may be reviewed and adapted as the needs of the team and organisation evolve.
