Job summary

Kent Community Health NHS Foundation Trust is seeking a dynamic nurse to join our Children's Therapies team in East Kent. You will assess, plan, implement and evaluate holistic care linked to prescribed care packages, while supporting supervision and development of staff.

The role includes leading care-coordination pathways, working with education and health partners for multi-agency service delivery, and contributing to governance and CPD. A driving licence is often required in this role.

Qualifications

  • Level 6 Degree or equivalent qualification or knowledge via experience plus nursing diploma.
  • Experience managing complex caseloads and mentoring or supervising others.
  • Experience working with children and their families.
  • Registered with the Nursing and Midwifery Council (NMC).
  • Strong communication skills, both written and spoken.
  • Customer-focused approach to provide the best possible patient experience.
  • Ability to use IT systems and databases.
  • Awareness of NHS developments and child health guidance.

Responsibilities

  • Provide assessment, planning, implementation and evaluation of holistic care linked to prescribed care packages or identified projects.
  • Support supervision and development of staff.
  • Act as lead and support for care coordinators, managing care coordination pathways.
  • Collaborate with education and health colleagues to support multi-agency service delivery.
  • Contribute to regular governance, CPD activities and ongoing staff induction.

We are seeking a dynamic and enthusiastic nurse to join our friendly team in East Kent. The Children's Therapiesservice provides Speech and Language Therapy, Occupational Therapy, Physiotherapy and Care Co-ordination services toCYP aged 0-19., o Provide assessment, planning, implementation and evaluation of holistic care linked to packages of prescribed care oridentified projects.oSupport in the supervision and development of staff.oActively work as a member of the children's therapies team to provide high quality care to service users.oAct as lead and support for care co-ordinators, managing the care co-ordination pathwaysWork with key colleagues (ie education and health) in order to support multi agency models of service delivery.The successful applicant will join our friendly therapies team. We pride ourselves on the support and induction offered tonew staff including; regular meetings with a line manager, access to supervision; in house and external training. We offerregular clinical governance days where all localities come together to share knowledge, skills and CPD experiences.   • You will have a Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing
  • You'll need to have experience of managing complex caseloads and supporting/ mentoring others
  • You will need to have experience working with children and their families.
  • You will be a registered health care professional with the Nursing and Midwifery Council.
  • You will have at least one year's experience of working as a Nurse or midwife., You'll need have sound communication skills in order to understand and communicate both verbally and in writing
  • You will need to have a customer focussed approach which is essential to; Provide patients, clients and their families with the best possible experience
  • You will need to be able to be able to use IT systems and databases.
  • Having an awareness of developments within the NHS would be an advantage as well as an understanding of National Service Frameworks
  • You will have knowledge of up-to-date children's health and well -being by using up- to- date evidence-based research and clinical effectiveness frameworks. You will fully understand the role and remit of a health care worker. About You Here's what you need to thrive in this role, You will have the ability to focus on the details and spot any potential problems.
  • You'll need to be adaptable in work as things change as part of working in a wider team.
  • A full, valid UK driving licence or a full driving licence issued by an EU country (not exchanged from a non-EU country)
