The aim of the Intermediate Care Lambeth is to provide community based care and rehabilitation to prevent unnecessary hospital admission, facilitate early hospital discharges and promote independence reducing reliance on long‑term social care services.
The successful applicant will work alongside Physiotherapists, Occupational Therapists, Social Workers, Social Care Assessors, Pharmacists, Doctors/GPs and Rehab Support Workers as part of the multi‑disciplinary team.
Main Duties of the Job
Professional / Clinical Responsibilities
- To be competent in using a wide variety of specialised assessment and treatment approaches with adults with a range of conditions, including those with severe physical and cognitive impairment, in order to formulate treatment plans, maximise rehabilitation potential and set goals.
- To be professionally and legally responsible for own practice and accountable for all aspects of treatment delivered to the client to ensure a high standard of clinical care.
- To work as an autonomous lone practitioner, case‑managing clients in the community and coordinating their care.
- To carry out timely assessment and rehabilitation of community clients to prevent hospital admission or re‑admission and facilitate early discharge home from hospital.
- To carry out clinical assessment of patients' conditions and intervention planning for management of blood pressure, pressure care, continence assessment, medication reconciliation, urgent wound assessment and falls prevention.
Management and Leadership
- To monitor the quality of nursing/rehabilitation services offered and contribute to the development and review of quality measures and performance indicators.
- To monitor the quality of services offered by the team and implement action plans to address areas of development under the direction of the Clinical Manager.
- To deputise in some tasks in the absence of the Senior Specialist Nurse if required.
Communication
- To be skilled in communicating, using both verbal and non‑verbal means, with clients, carers and families who may have a range of physical disabilities or other barriers to understanding.
- To offer advice and communicate written and verbal information to clients/carers and agencies, including other health professionals, regarding a client's management.
- To communicate effectively with clients, carers and family members and develop effective working relationships with the multidisciplinary team and external agencies.
Education, Training & Mentorship
- To educate referrers and clients about the services the team offers; to induct, supervise, appraise and teach students on placements within the team; to teach and assess students and rehab support workers.

Person Specification
Essential Skills, Knowledge & Ability
- Demonstrate highly developed verbal, non‑verbal and written communication skills in English.
- Excellent interpersonal and negotiating skills, including observation, listening and empathy skills.
- Ability to use an evidence‑based approach including clinical outcome measures to evaluate outcomes.
- Ability to work as an autonomous practitioner and make sound clinical judgments and decisions regarding patient care.
- Ability to manage a complex and constantly changing workload of clinical and non‑clinical duties.
- Ability to plan and organise the provision of care to the team's caseload of patients.
Essential Previous Experience
- Experience of working with professionals from other disciplines.
- Experience of working with adults and older people with chronic conditions and complex co‑morbidities.
- Experience of teaching and assessing students and rehab support workers.
- Experience of undertaking initial assessment.
- Ability to understand and contribute to a rehabilitative approach with clients.
Desirable Experience
- Experience of supervising other members of staff.
- Experience of working in a rehabilitation, reablement or urgent response setting.
Essential Qualifications
- Recognised Nursing degree qualification or equivalent.
Desirable Qualifications
- Member of relevant Clinical Interest Group.
- Recognised post‑graduate qualification in specialist clinical area.
Essential Registration
- Nursing and Midwifery Council State registration.
