Specialist in Frailty

Overview

We are seeking to appoint a substantive Specialist in the Community, Frailty & Therapy Care Group to join a team of Consultants on the south bank. This newly formed role will manage frailty within urgent and emergency care across our UEC footprint, with two specialist posts – one for Scunthorpe General Hospital and one for Diana Princess of Wales Hospital at Grimsby.

The successful applicant will be appointed as an Honorary Senior Lecturer at Hull York Medical School (HYMS), recognising the contribution to teaching and research.

Responsibilities
  • Clinical assessment: Provide expert clinical assessment and decision-making for frail older adults at the front door of acute care (e.g., Emergency Department, Same Day Emergency Care, Frailty wards).
  • Comprehensive Geriatric Assessment (CGA): Lead or support the delivery of CGAs to ensure rapid and holistic assessment of frailty, medical needs, functional status, cognition and social factors.
  • Multidisciplinary collaboration: Work closely with nursing staff, allied health professionals, geriatricians, social workers and community services to enable timely, person‑centred care.
  • Admission avoidance: Develop and implement plans to avoid unnecessary hospital admissions, facilitating safe discharge or transfer to appropriate community‑based services.
  • Pathway development: Contribute to the development and improvement of frailty pathways, including rapid access clinics, discharge‑to‑assess models and integrated care solutions.
