Job summary

A prominent healthcare provider in Scotland is seeking an experienced Community Physiotherapist to deliver specialized physiotherapy services. We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.

The shift pattern for this post is 8:30 - 16:30

This post is part of the Community Hospital Discharge Team within West Dunbartonshire HSCP providing community rehabilitation and/or Advanced Care Planning. It offers the opportunity to work as a community physiotherapist with all adult care groups and to provide direct clinical skills which encompasses highly complex musculoskeletal, orthopaedic, respiratory, neurological and other multi‑pathology conditions.

Responsible for delivering a specialised physiotherapy service to patients/clients living within West Dumbarton Health and Community Care Service catchment area.

To perform advanced assessment of patients including those with highly complex presentations and from this formulate a diagnosis and treatment plan and deliver as an autonomous practitioner. Participate in Anticipatory Care Planning as required with the aim to minimise avoidable hospital admissions and maximise individual’s ability to remain safely at home.

A member of a multi‑disciplinary team (MDT) which provides highly specialised assessment, rehabilitation and care to patients within the community with the aim to minimise avoidable hospital admissions and maximise individuals’ ability to return/remain safely at home.

Provide physiotherapy advice and direction to other members of the MDT.

Supervise less experienced Physiotherapists as well as Support Workers and students on clinical placement.
