NHS Forth Valley is seeking a Specialist Clinical Pharmacist for a nine-month fixed-term/secondment post to develop specialist clinical, leadership and service improvement skills across Medicine and Care of the Elderly.

The role involves flexing across Medicine and Care of the Elderly initially, then focusing on Medicine to deliver a range of specialist clinical pharmacy services and contribute to rotas, teaching and service development.

Qualifications

Well-rounded clinical practitioner with post-registration foundation training.
Independent prescribing experience desirable, development opportunities may be available.

Responsabilités

Deliver specialist clinical pharmacy services to Medicine and Care of the Elderly inpatient areas.
Participate in ward rounds, clinical reviews and medicines-related decision-making.
Provide medicines reconciliation, clinical review, prescribing support and medicines optimisation.
Support safe and effective prescribing and deprescribing, including polypharmacy reviews where appropriate.
Deliver education and training to rotational pharmacists, pharmacy technicians and students.
Contribute to clinical governance, audit, quality improvement and service development.
Participate in weekend, public holiday and on-call services.
