Overview

Senior Social Worker - Specialist CHC - Medway Council. 3D Recruit invites you to join Medway Council's Adult Social Care service in a hybrid role to lead complex assessments, shape care planning, and make a genuine difference to people across the community.

Key Responsibilities
  • Lead and coordinate CHC assessments and reviews (virtual and face-to-face)
  • Manage a clinical caseload from assessment to resolution
  • Collaborate with stakeholders to arrange care packages and placements
  • Provide expert advice on CHC, Section 117, and joint funding pathways
  • Maintain accurate records and support data reporting and governance
  • Contribute to training, induction, and continuous improvement initiatives
What We're Looking For
  • Registration with Social Work England (SWE) or Nursing and Midwifery Council (NMC)
  • Substantial post-qualification experience in social work or nursing
  • Strong knowledge of CHC, Mental Capacity Act (MCA), and consent processes
  • Excellent communication, report writing, and IT skills
  • Ability to work independently and manage a dynamic caseload
  • Full UK driving licence and access to transport
Desirable
  • Experience with Section 117 aftercare
  • Background in mental health, learning disabilities, or older adult services
