Berkshire Healthcare NHS Foundation Trust is recruiting a Specialist Cancer Pharmacy Technician to deliver medicines management across Cancer Services and support patients throughout their treatment pathway. The role includes medicines reconciliation, review of records, and informing safe treatment planning.

You will work with oncology, haematology, medical and nursing teams to ensure safe treatment pathways, assist with discharge planning, medicines supply, and coordinate with GPs and community

Qualifications

• Undertake medicines reconciliation and drug history taking.
• Identify discrepancies and medicines-related risks.
• Collaborate with oncology and haematology teams.

Responsabilités

• Deliver specialist medicines management service across Cancer Services.
• Support patients through their cancer treatment pathway.
• Assist with safe discharge and medicines supply coordination.
• Develop and implement homecare pathways for oral and subcutaneous SACT.
