Job description
Overview

Lambeth Single Point of Access (SPA) are a team of mental health professionals, support workers and social workers who help you access one of three Living Well Centres. These offer short and longer term support from our teams of clinicians, social workers, people with lived experience and support workers, as well as other services and community groups who can support you depending on your need.

We are a part of the Lambeth Alliance and have a close interface with a variety of SLaM services, other statutory organisations (including social care) and voluntary sector partners. The Lambeth Living Well Network (LWN) Alliance brings together Certitude, Lambeth Clinical Commissioning Group, Lambeth Council, South London & Maudsley NHS Foundation Trust and Thames Reach to deliver an integrated mental health system focused on people’s strengths and whole health and wellbeing. The SPA is the front door for mental health services in Lambeth.

We are looking for dynamic and enthusiastic candidates with strong leadership skills and a commitment to delivering high standards of care. Please note this vacancy may close early once capacity has been met.

Responsibilities
  • The post holder will work with the Team Lead and Clinical Expert to support and advise on the delivery of high-quality safe care.
  • You will coordinate multidisciplinary working, maintaining excellent communication and working relationships with all team members and other services within the Alliance to ensure effective interface and continuity of care between disciplines and services.
  • You will have expertise in leading conversations about people’s health and wellbeing and be able, within the context of the triage proforma, to carry out mental health and risk assessments and provide general health, wellbeing and self-management advice.
  • Based on the triage discussion, you will give clear direction on what clinical intervention is indicated and whether this is advised within a primary shared care model or within secondary care.
  • You will support the development of joint working opportunities and protocols alongside other Network organisations for people who do not need a more formal offer, and you are required to encourage collaboration and cross-organisational working with wider community services as a member of the Alliance.
  • If you are a skilled mental health nurse with effective and compassionate communication skills, this is a fabulous opportunity to join our team.

  • Car lease options and keyworker housing on selected sites.
  • NHS discounts up to 10% from various retail brands.
  • Counselling services, wellbeing events, long service awards, cycle to work scheme, season ticket loan and childcare vouchers.
  • Staff restaurants and other staff benefits.
