Overview

The SPA Clinical Triage Nurse receives referrals from diverse services, triages them, and ensures timely, appropriate service connections. The role supports the Virtual Frailty Ward during daily MDT meetings, monitors the Health Call system for heart failure patients, escalates out-of-range observations, and provides Palcall telephone support and signposting to individuals within the last 12 months of life.
