A public health service provider in Billingham is looking for a SPA Clinical Triage Nurse to efficiently triage referrals and support the Virtual Frailty Ward. The role requires excellent communication skills and the ability to handle multiple tasks. Candidates will receive DBS checks and may be eligible for visa sponsorship. This position offers full-time hours with flexible options available.
Role: The SPA Clinical Triage Nurse receives referrals from a diverse range of people and services, triages those referrals to ensure they reach the correct service in the shortest time, supports the Virtual Frailty Ward during its daily multidisciplinary team meetings, monitors the Health Call system used by heart failure patients to ensure out‑of‑range observations are escalated and acted upon, and provides telephone support and signposting through the Palcall service for people within the last 12 months of life.
Key responsibilities: • Receive and triage referrals from a variety of sources. • Ensure referral flow to the appropriate service is timely. • Monitor and respond to Health Call system alerts for heart failure patients. • Support the Virtual Frailty Ward’s MDT meetings. • Deliver telephone support and signposting via Palcall.
Qualifications and skills: • IT literate with the ability to use digital systems efficiently. • Excellent multi‑tasking ability. • Advanced communication skills with patients, carers and other health professionals.

Successful candidates who are new to North Tees and Hartlepool NHS Foundation Trust and who require DBS clearance for the post are required to pay the cost for their DBS certificate. The cost is deducted from your first month's pay. The DBS check will be undertaken during the pre‑employment screening process.
Applicants must provide references covering their last three years of employment history, excluding personal references. All referees must provide email addresses on the application form.
The advertised hours are up to full time (37.5 hours per week). Applicants seeking to work less than full time are welcome to apply and requests will be considered in line with the post's needs.
