3 x Social Worker – Initial Assessment Team (CAP Assessment / CAP MDT Rapid Response)

Location: Guildhall, Swansea

Contract: Full-time | Hybrid working

Description

The City and County of Swansea are seeking three experienced and dedicated Social Workers to join our Initial Assessment Team – two positions within the CAP Assessment Team and one within the CAP MDT/Rapid Response Team. These roles offer a fantastic opportunity to work within a fast-paced, supportive environment where you will play a crucial role in assessing and supporting adults in need of care and protection. The teams operate closely together under the same management structure, so there may be some flexibility required across both functions depending on service demand. This is a hybrid role, combining office-based work at Guildhall, Swansea with community visits and remote working. Flexibility is encouraged as long as visits and statutory duties are completed in a timely manner.

Responsibilities
  • Undertake comprehensive assessments and provide case management for adults in need of care and support.
  • Carry out statutory duties, including safeguarding investigations and court work, ensuring compliance with relevant legislation.
  • Promote strength-based, person-centred outcomes that support independence and community inclusion.
  • Develop and maintain effective working relationships with health, housing, and community partners to ensure coordinated multi‑agency support.
  • Provide timely interventions and support to adults at risk, carers, and families, ensuring their wellbeing and safety.
  • Maintain accurate, up-to-date records and contribute to the continuous improvement of the service.
Requirements
  • Qualified Social Worker registered with Social Care Wales.
  • Experience in adult social care, ideally within assessment, community, or hospital discharge settings.
  • Good understanding of the Social Services and Wellbeing (Wales) Act 2014 and related legislation.
  • Ability to work independently and as part of a multidisciplinary team.
  • Strong communication, assessment, and organisational skills.
  • Willingness to work flexibly and travel for home or hospital visits as needed.
