Purpose of the Role

To provide high-quality social work services that assess needs, plan and coordinate interventions, and work collaboratively with individuals, carers, and partner agencies to achieve positive outcomes.

What You Will Do
  • Undertake proportionate assessments and create outcome-focused care and support plans.
  • Carry out risk assessments and implement safeguarding measures when required.
  • Provide direct support, guidance, and advocacy for individuals and families.
  • Collaborate with partner agencies (health, education, housing, police, voluntary sector) to deliver integrated care.
  • Maintain accurate and up-to-date records in line with professional and statutory standards.
Person Specification
  • Qualified Social Worker (Degree or Diploma) with current registration with Social Care Wales.
  • Experience in statutory social work, including safeguarding, assessments, and case management.
  • Strong knowledge of Welsh legislation and guidance, including the Social Services and Well-being (Wales) Act 2014.
