We are seeking dedicated and skilled Social Workers to join our Through Age Integrated Assessment Team in West Wales. This unique team works across all age groups, providing holistic assessments and tailored interventions for children, young people, adults, and families.

About the Role

As a Social Worker in the Through Age Integrated Assessment Team, you will:

  • Conduct comprehensive assessments for individuals across all age ranges and be comfortable with a case-load of S47s.
  • Work collaboratively with professionals from health, education, housing, and other key sectors to ensure integrated, person-centred care.
  • Develop and implement care plans that are tailored to meet the unique needs of each individual or family.
  • Offer early intervention, crisis support, and long-term planning for vulnerable individuals.
  • Advocate for service users, ensuring their voices are heard and they are empowered to make informed decisions.
