A healthcare provider in Telford is seeking a Social Worker to provide oversight and delivery of planned recovery-focused care. This role involves assessing and planning care in collaboration with service users and other professionals while working within a multi-disciplinary team. The ideal candidate will have experience in mental health settings and offer care focused on recovery and support for individuals in crisis.

The team is based at Hall Court in Telford and provides mental health assessment, treatment and support to people in the area of Telford & Wrekin. As an integral team in the Urgent Care Pathway we work 24 hours a day, 7 days a week. We work with people who are experiencing a mental health crisis, require extra support alongside their regular support network or who can have the same treatment provided at home that they would receive in a Mental Health Hospital.
We are a team that consists of Band 5 and 6 Nurses, Social Workers, Band 3 Support Workers, Non-Medical Prescribers, Medical Staff, Occupational Therapists, Administrators and Business Support Staff. As we work with people with all types of mental health presentations, we need to have a range of skills and interests that create a strong and responsive team.
We are looking to recruit a Social Worker who will be responsible for the oversight and delivery of the provision of planned recovery focused care working in partnership with Service-users carers and other professionals.
You will be responsible for working collaboratively with other services to provide seamless care for service-users on defined care-pathways and within defined care clusters.
MPFT will offer a welcome bonus of 10% of first year salary - at 50% on commencement of employment and 50% following 12 months employment.
