• Manage a diverse caseload across supported living services in and out of the Reading Borough.
• Collaborate with the project Occupational Therapist, care providers, Housing Services, and health colleagues to ensure integrated support.
• Engage in regular site visits and office work to maintain high-quality service delivery.
• Develop and review personalised, strength-based support plans that focus on positive outcomes.
• Implement innovative, tech-based solutions and reablement approaches to support independence.
• Work closely with individuals, their families, and providers to ensure services are co-produced and align with the REACH standards.
