Undertake Court of Protection, safeguarding, Care Act assessments, and Mental Capacity Act evaluations.

Utilise your knowledge of Liquid Logic to support and streamline case management.

Collaborate effectively with individuals, families, health professionals, and education partners.

Support individuals transitioning from hospital or educational settings into supported or independent living.

Promote smooth and effective transitions from children’s services into adult services.

Apply creative and person-centred approaches to engage and support service users.

Ensure thorough understanding and correct application of the Care Act and Mental Capacity Act in all decisions.

Provide proactive support for commissioned services and direct payments.

Participate actively in team huddles and contribute positively to the team’s shared goals.
