What you will do
  • Undertake detailed assessments of individuals’ needs, risks, and strengths in accordance with relevant legislation such as the Care Act 2014 and the Mental Capacity Act 2005. Emphasise a strengths-based approach to empower individuals.
  • Develop and implement personalised care and support plans in collaboration with service users, families, carers, and other professionals to ensure the delivery of effective services.
  • Respond to safeguarding concerns by conducting thorough risk assessments and investigations, ensuring timely and appropriate interventions.
  • Advocate for service users’ rights and preferences, ensuring their voices are integral in all decision-making processes. Work with service users to develop care plans that maximise independence.
  • Conduct regular reviews and update care plans as necessary, ensuring continued appropriateness and effectiveness. Liaise with external agencies for ongoing support.
  • Collaborate effectively within a multi-disciplinary team, including healthcare professionals, housing services, and community organisations. Participate in care coordination meetings.
  • Maintain accurate documentation and reporting in line with legal, regulatory, and organisational requirements. Prepare reports for care reviews and professional meetings as needed.
  • Identify and manage risks to individuals’ safety and wellbeing, undertaking Mental Capacity Assessments and making best interest decisions when necessary.
