Job summary

Sanctuary Personnel Limited is seeking a Social Worker for the Leaving Care Team in Hackney, London (UK). The role offers ongoing work with a choice of full-time or part-time hours at a rate of £37.52 per hour.

You will support care-experienced young people as they transition to independence within Children’s Social Care. You will manage caseloads, develop and review Pathway Plans, build strong relationships with young people, and coordinate access to housing, education, employment and health

Qualifications

  • Degree or Master's Degree in Social Work or an equivalent recognised qualification is required.
  • Successful completion of the ASYE is desirable.
  • Evidence of continuing professional development is essential.

Responsibilities

  • Manage a caseload of care-experienced young people and conduct statutory assessments, interventions and reviews.
  • Develop, implement, and regularly review Pathway Plans focused on the unique needs and aspirations of each young person.
  • Build and sustain effective professional relationships with young people, ensuring their voices guide planning and decision-making.
  • Facilitate access to suitable accommodation, education, employment, training, and health services for young people.
  • Conduct risk assessments and safeguarding enquiries, maintaining adherence to statutory duties and local procedures.
