Overview

We've got an exciting opportunity for you as a Social Worker in Adult Social Care. Our mission is to empower adults with learning disabilities to live independently as possible, and we need your expertise to make it happen. This role will challenge you in a dynamic and diverse environment, helping you expand your skills and knowledge in the practice of social care. Your duties and responsibilities will include:



Responsibilities


  • Collaboration with multidisciplinary teams, healthcare professionals and community partners to enable a holistic approach to meeting the needs of the person

  • As a Social Worker within the Learning Disabilities team, you will have an essential role supporting people to live their best lives and maximise their independence. You will be working with health colleagues and community services to ensure people have the required level of support to meet their needs.


This position offers an excellent opportunity for individuals with outstanding communication skills and a compassionate manner, who are capable of effectively managing fast paced work. Candidates should have the experience and ability to work under-pressure in a fast paced environment, in order to manage competing demands. We encourage applications from candidates with experience in adult social care and those passionate about delivering person-centred care and prevention strategies to manage risk.



Qualifications


  • Qualified Social Worker with current registration with Social Work England and eligibility to work in the UK.

  • 2 years post ASYE qualifying experience working in adult services, assessing and supporting individuals with diverse needs and successful completion of their ASYE.

  • Sound knowledge of relevant legislation, including the Care Act 2014 and Mental Capacity Act 2005 as well as social work theories

  • Experience in Adult Safeguarding work

  • Strong assessment, outcome focussed care planning, and risk management skills

  • Ability to travel across the borough to visit individuals, occasionally being required to undertake visits out of borough


Please note: Social Workers may be required to travel outside the Borough to visit clients as part of their caseload. Further details regarding the caseload and travel expectations will be provided during the interview process.
