About the role

We are delighted to recruit for a permanent Social Worker to join our Charing Cross Hospital Discharge Team. The team delivers a hosted hospital discharge model, providing assessment and discharge support to residents of Westminster, Royal Borough of Kensington and Chelsea and London Borough of Hammersmith and Fulham. You will work with Acute Hospital trusts, Community Health Teams, Community Independence Service, local CCGs, Continuing Health Care Teams and other partners to ensure safe and timely discharges.

Responsibilities
  • Complete Care Act assessments and ensure adherence to the Care Act 2014 and Mental Capacity Act 2005.
  • Facilitate safe discharges and partner with NHS/Health colleagues across hospital trusts and community teams.
  • Support the senior practitioner and Team Manager with discharges, providing supervision to student social workers.
  • Maintain excellent standards of Social Work practice underpinned by the Care Act 2014 and the Mental Capacity Act 2005.
