Job summary

Qualifications

• Qualified and registered Social Worker with Social Work England.
• Experience in adult social care or related areas.
• Experience with hoarding is a plus.

Responsibilities

• Review referrals, prioritise cases, and plan visits.
• Assess needs and create person-centred care plans.
• Collaborate with NHS partners and voluntary sectors to deliver joined-up care.
• Contribute to safeguarding and maintain clear records using digital systems.
