Job summary

Hampshire County Council Continuing Healthcare (CHC) team seeks a Social Worker to manage a diverse caseload and work across adults with mental health and disability needs. You’ll collaborate with care teams to support NHS CHC assessments and onward care planning after hospital discharge.

Applicants should have UK social care experience, a Social Work qualification with SWE registration, and be committed to continual professional development.

Qualifications

• Social Work qualification and SWE registration.
• UK-based experience within Adult Social Care.
• Understanding of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (July 2022).

Responsibilities

• Supporting individuals with their onward care journey following a Discharge to Assess period from hospital.
• Undertaking assessments in line with the Care Act 2014.
• Engaging with multidisciplinary teams to support completion of NHS CHC assessments.
• Collaborating with individuals and their families to transition them to suitable care provision following assessment.
• Undertaking mental capacity assessments.
• Managing challenging and complex cases.

As a registered Social Worker in the Hampshire County Council Continuing Healthcare (CHC) team, you'll work within a multidisciplinary environment collaboratively with your social care colleagues and other professionals in the varied and complex field of NHS CHC. You'll be supported to manage a diverse caseload, working across all care groups including older and younger adults, with mental health concerns and physical and learning disabilities.,

• Supporting individuals with their onward care journey following a Discharge to Assess period from hospital.
• Undertaking assessments in line with the Care Act 2014.
• Engaging with multidisciplinary teams to support completion of NHS CHC assessments.
• Collaborating with individuals and their families to transition them to suitable care provision following assessment.
• Undertaking mental capacity assessments.

  Social Work qualification and registered with Social Work England (SWE). We would welcome applicants wanting to complete their ASYE.
• UK based experience within Adult Social Care.
• Demonstratable experience of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care July 2022 (Revised).
• A sound understanding of how to apply a strengths-based approach.
• Understanding of social work issues, legal frameworks and social services' statutory duties.
• Commitment to demonstrating our values and behaviours, showing compassion, empathy and respect., You will be required to travel to various locations within Hampshire, and it is essential that you have a current drivers' licence and access to a reliable vehicle on a regular basis.
