Social Worker - Community Mental Health Team

Location: Central Clinic, Orchard Street, Swansea

Contract: Full-time (37 hours per week)

Hybrid working: Combination of office, home-based, and community visits

Description

The City and County of Swansea are seeking a dedicated and experienced Social Worker to join the Community Mental Health Team (CMHT). This is an exciting opportunity to make a real difference in the lives of individuals experiencing serious mental health challenges, helping them to live as independently and meaningfully as possible within the community. You'll be part of a multi‑disciplinary, recovery‑focused team, working collaboratively with health professionals, carers, and families to provide integrated and person‑centred care.

Responsibilities
  • Undertake statutory social work duties in line with the Social Services and Well‑being (Wales) Act 2014 and Mental Health Act 1983.
  • Assess the needs and circumstances of individuals and families, developing tailored care and support plans.
  • Act as Care Co‑ordinator, ensuring care packages are well planned, monitored, and evaluated.
  • Attend Mental Health Tribunals and contribute to care reviews and planning meetings.
  • Carry out risk assessments and take appropriate steps to safeguard vulnerable individuals.
  • Work collaboratively with internal and external agencies to promote recovery and maximise service user participation.
  • Maintain accurate, timely, and confidential case records.
Requirements
  • Qualified Social Worker registered with Social Care Wales.
  • Proven experience in mental health social work, preferably within a community or multi‑agency setting.
  • A valid driving licence and ability to travel as part of the role.
  • Knowledge of WCCIS (Welsh Community Care Information System) is advantageous.
