Job summary

Brent Council is seeking Social Workers for the Children with Disabilities (CWD) team to assess and plan care for children with severe disabilities.

You will coordinate services, advocate for families, and ensure safeguarding throughout the care journey, including transition planning at age 18. The role emphasizes collaborative practice within Brent’s diverse community and strong commitment to outcomes for children with disabilities.

Qualifications

• SWE registration and post-qualifying experience in children’s social care (CWD experience highly desirable).
• Knowledge of the Children Act and child care law; confident practical application.
• Skilled at assessing complex needs and writing clear, evidence-based plans and reports.

Responsibilities

• Carry out comprehensive assessments of children with disabilities and their families.
• Develop and implement tailored care plans meeting children’s needs and promoting outcomes.
• Arrange and coordinate services such as home care, equipment, direct payments and short breaks.
• Advocate for the child and family to access entitled resources and support.
• Safeguard children by identifying risks and undertaking required assessments (Section 17/47).
• Support transition planning from children's to adult services at age 18.
• Work with multi‑agency partners to provide joined-up care.
