Purpose of the Role

As a Social Worker in the CWD Team, you’ll provide high-quality social work services to children and young people with disabilities and their families. You’ll assess, plan, implement, and review tailored support packages that enhance each child’s wellbeing, safety, and development. Your work will be guided by statutory duties, best practice standards, and the policies of Halton Borough Council.

Key Responsibilities

Undertake statutory assessments in accordance with relevant legislation, identifying the needs of children and their families using evidence-based practice.

Develop and implement care and support plans that promote independence and safeguard vulnerable children.

Maintain accurate, comprehensive case records and documentation in line with statutory requirements.

Participate in child protection investigations, strategy discussions, and planning meetings.

Offer practical support and guidance to families, coordinating and reviewing packages of care.

Support children and families through transition planning from children’s to adult services.

Work collaboratively with partners across education, health, and voluntary sectors to ensure joined-up service delivery.

Contribute to local policy development and share insights to drive service improvement.

Uphold Social Work England professional standards and engage in continuous professional development (CPD).

Qualifications and Experience

Degree in Social Work (or equivalent qualification).

Registration with Social Work England.

Experience within children’s social care, ideally in a CWD context.

Strong understanding of relevant legislation and effective case management skills.

Experience working with children who have learning disabilities, autism, complex health needs, or sensory impairments.

Knowledge of short breaks, direct payments, and personal budgets.

Familiarity with person-centred and partnership approaches.
