Job summary

Jacobs Neurological Centre is seeking a Social Worker Assistant to support assessments and social arrangements for adults with neurological conditions. You will liaise with families, external agencies, and the care team to progress outcomes and promote independence.

The role requires understanding of the Mental Capacity Act and safeguarding, plus strong communication skills. 37.5 hours weekly, with training opportunities available through Elysium Healthcare.

Qualifications

• Excellent communication and interpersonal skills are essential.
• Ability to liaise with service users and external agencies.
• Understanding of Mental Capacity Act and safeguarding vulnerable adults is required.

Responsibilities

• Carrying out assessments with service users as directed to identify outcomes.
• Liaising with external agencies to facilitate arrangements.
• Assisting service users with budgeting and financial arrangements.
• Support the Social Worker through meetings and assessments.
• Cover meetings, calls and interviews when the Social Worker is unavailable.
