We are seeking dedicated and skilled Social Workers to join a stable and well run Assessment Team in North Wales. 3 days in the office applies as the team work closely together to foster support and work as a collective.

Long Contract available to see you past Christmas and New Year - with 6 months on offer (subject to renewal)!

About the Role:
  • Conduct comprehensive assessments for individuals across all age ranges and be comfortable with a case-load of S47s.
  • Work collaboratively with professionals from health, education, housing, and other key sectors to ensure integrated, person-centred care.
  • Develop and implement care plans that are tailored to meet the unique needs of each individual or family. Offer early intervention, crisis support, and long-term planning for vulnerable individuals.
  • Advocate for service users, ensuring their voices are heard and they are empowered to make informed decisions.
