Job summary

Service Care Solutions is seeking an experienced Adults Social Worker (Level 2) for the Physical Disabilities & Older People team in Norwich, Norfolk. The role is a temporary agency contract initially for 12 weeks, with a focus on person-centred and strengths-based support in the community.

You will work with health professionals and partner agencies to promote independence and wellbeing, handling complex cases and ensuring timely documentation and professional judgment from day one.

Qualifications

• Recognised Social Work qualification (Degree, Diploma or equivalent).
• Registration with Social Work England is required.
• Experience in Adults Social Care, preferably with older people or physical disabilities.
• Knowledge of Care Act 2014, Mental Capacity Act 2005 and safeguarding legislation.
• Strong communication and organisational abilities with multidisciplinary teams.

Responsibilities

• Manage a varied community caseload including adults with disabilities and older people.
• Carry out comprehensive assessments under the Care Act 2014.
• Complete Mental Capacity Assessments per the Mental Capacity Act 2005.
• Undertake safeguarding enquiries and manage risks.
• Develop person-centred, strengths-based support plans.
• Collaborate with health professionals, families and external agencies.
• Identify early intervention and community-based support opportunities.
• Maintain accurate case records and documentation.
• Apply professional judgement to complex cases.
