What you will do:
Deliver high-quality social work practice in line with the Social Services and Well-being (Wales) Act 2014.
Safeguard and promote the wellbeing of vulnerable adults, ensuring their safety, dignity, and rights are upheld.
Manage complex cases through comprehensive risk assessments and robust care planning.
Work collaboratively with multi-disciplinary teams and external agencies to achieve positive outcomes for service users.
Assessment & Care Planning: Undertake statutory assessments, develop outcome-focused care plans, and complete regular reviews.
Risk Management: Balance safeguarding responsibilities while promoting independence and choice.
Direct Practice: Support individuals with physical disabilities, mental health needs, substance misuse issues, and those transitioning from children’s to adult services.
Supervisory Duties: Provide guidance to other Social Workers and act as deputy in the absence of the Team Manager.
Registered with Social Care Wales.
Minimum of 3 years’ post-qualification experience for progression or secondment opportunities.
Strong knowledge of relevant legislation, safeguarding frameworks, and person-centred practice.
Ability to manage complex caseloads with high standards of written and professional practice.
