Qualifications

• Must be a qualified Social Worker.

Responsibilities

• Support people in their own communities.
• Help reduce hospital admissions through timely intervention.
• Complete Care Act assessments and reviews.
• Arrange and coordinate appropriate care and support packages.
• Act as Safeguarding Enquiry Officer.
• Undertake Mental Capacity Assessments and Best Interest decisions.
• Work collaboratively with professionals across health and social care.

👩⚕️ Social Worker - Older People's Service (Level 10)

📍 Location: East Herts, Broxbourne & Welwyn Hatfield

🏢 Directorate: Adult Care Services

👥 Service Area: East and North Older People's Service

💰 Pay Rate: Up to £34.00 per hour (depending on experience)

🔄 Working Pattern: Hybrid working

🕘 Hours: Mon-Thurs 9:00-17:30 | Fri 9:00-16:30

🔐 DBS Required: Yes
