Overview

Are you a dedicated and passionate Social Worker looking to make a positive impact in the lives of young adults within our community? West Berkshire Council's Adult Social Care Transitions Team is seeking a skilled Social Worker to join our dynamic and supportive team, reporting to the Team Manager.

Responsibilities
  • Conducting assessments and reviews of individuals' social care needs, ensuring their rights and preferences are upheld.
  • Providing information, advice, and guidance to service users and their families to promote independence and wellbeing.
  • Collaborating with other professionals and agencies to ensure coordinated and holistic care delivery.
  • Implementing appropriate interventions and support plans to meet identified needs.
  • Advocating for service users and supporting them in accessing relevant services and opportunities.
  • Maintaining accurate and timely documentation, including case records and reports.
Skills and Attributes
  • Qualified Social Worker with a recognised social work degree and valid Social Work England registration.
  • Experience working in an adult social care setting, with a sound understanding of relevant legislation and frameworks.
  • Strong assessment and intervention skills, with the ability to work collaboratively with individuals and multidisciplinary teams.
  • Excellent communication and interpersonal skills, demonstrating empathy and respect towards service users.
  • Ability to manage competing priorities and work effectively under pressure, demonstrating resilience and adaptability.
  • Knowledge of safeguarding principles and the ability to identify and respond to risks.
  • Driving licence and access to a vehicle for work purposes.

Contract and Discussion
