Social Worker (Level 2)
Job Code: 9140 | Permanent contract | £39,862 to £41,771 per annum (Scale J) | 37 hours per week | Julian Hospital, Norwich

Overview

We are looking for a qualified Social Worker to join Adult Social Services at Julian Hospital, an adult care facility for people aged 65+. You will support patients from admission to discharge, working closely with a multidisciplinary team to promote wellbeing and mental health in later life.

Responsibilities
  • Provide social work assessments and care planning for admissions.
  • Support patients through treatment and discharge processes.
  • Collaborate with medical and therapeutic staff to ensure holistic care.
  • Develop specialist knowledge in older person mental health.
  • Maintain accurate case records and documentation.
  • Act as a point of contact for families and caregivers.
Qualifications & Requirements
  • Registered with Social Work England (SWE) and practising with a recognised social work organisation.
  • Experience in adult social care and mental health services is desirable.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
