The role involves supporting residents through discharge processes and handling complex cases in a collaborative environment. This is a full‑time, permanent role with hybrid working, and a salary of ASYE £36,071 per annum. Social Worker £38,192 - £42,012 per annum.

This is an excellent opportunity for an enthusiastic Social Worker to join our Hospital and Community teams.
As a Social Worker within these teams you will be helping to support adults to live independently and safely by assessing needs, coordinating care, and safeguarding wellbeing. Work closely with individuals, families, and partner agencies to promote dignity, choice, and empowerment.
You will be supported with your caseload by a senior Social Worker and a Team Manager.
