In this role , you will support adults, carers and families to achieve outcomes that matter to them. Working creatively with people , communities and partners, you will promote independence, choice, safety and wellbeing while delivering our statutory responsibilities to a high standard. We are looking for practitioners who are curious, compassionate and confident in using evidence, professional judgement and community assets to improve outcomes.
