About The Post-Mental Health Team - Adult Social Care and Health

We are seeking dedicated and motivated individuals to join our Mental Health Team as Assistant Social Work Practitioners. Both full-time and part-time positions are available.

Key Responsibilities
  • Conduct comprehensive assessments and reviews of service users and carers, ensuring compliance with statutory requirements.
  • Identify risks, needs, and appropriate interventions, promoting independence through a strengths-based approach.
  • Develop and implement innovative care packages, including preventative work and risk assessments, utilizing community resources such as reablement and telehealthcare.
  • Maintain up-to-date knowledge of relevant legislation, including the Mental Health Act, Mental Capacity Act, Deprivation of Liberty Safeguards, Care Act (with Making Safeguarding Personal), and the Human Rights Act.
  • Collaborate effectively with individuals, families, carers, and professionals across health, housing, and partner organizations.
