Social Work Advanced Practitioner – Adults Hospital Team

We have an exciting opportunity for an Advanced Practitioner within Adults Hospital Team in Barnsley, earning £37 per hour, hourly. This is an ongoing contract for a full-time position. You will work in collaboration with health professionals across various discharge pathways, ensuring a seamless and swift flow for patient transitions.

As a pivotal member of the team, you will utilise your prior experience at an Advanced Practitioner level to support and lead social care staff with a deep understanding of legislative frameworks and complex case management. You’ll be the guiding force in critical decision-making processes.

What you will do:

• Collaborate with another Advanced Practitioner and the Team Manager to oversee a team of 22 Social Workers and Assistant Social Care Practitioners.
• Manage and monitor daily operations, including dealing with referrals, assigning tasks, and responding to staff inquiries.
• Attend daily meetings with health partners and ensure timely supervision of staff.
• Audit and authorise Care Act assessments and support plans, ensuring compliance and quality.
• Manage care resources and budgets effectively, ensuring financial efficiency.
• Maintain a small, manageable caseload dealing with individuals with multifaceted health and social care needs.
